What You Need to Enroll in a Health Plan

Collect the following documents that you will need to enroll in your health plan.

Proof of U.S. citizenship Social security numbers or copy of U.S. passport for all family members
Proof of the state you live in Driver’s license, housing lease or utility bill
Earnings for all family members W-2 forms or pay stubs, unemployment/disability, social security/pension/retirement income, or a copy of your most recent tax return
Current Health Insurance (if any) Policy numbers for any current health insurance and info about insurance if it is offered through your job
Immigration status A permanent resident card, work authorization and an immigrant Visa are among the documents you may use. (See healthcare.gov for the full list.)
Method of payment for an insurance plan Be prepared to select a billing method such as credit card or bank draft that you will pay directly to the health insurance company, not the Marketplace
